The proforma for uploading the details of GDS official information for Contributory Insurance Health Scheme is attached in the following link. Please update the information immediately as it is to be required to be submitted to Circle Office very urgently.
1. After entering all data related to your office please press Ctrl + S.
2. Please make sure that the data entered are correct before saving it.
3. Don't enter the same data more than once.
